  		Amendment to Birth Certificate for Mattie Frank Smith The original birth certificate had four main areas to be corrected. (1) date of birth was incorrect and had no year of birth. (2) had no first name for baby Smith (3) Corrected the full name of father (4) mothers name was not on the original certificate. Mattie Frank Smith was my grandmother and she applied for her social security number in 1952. By the time she got her social security card, the town of Soda, Texas, no longer existed. Her aunt, Mrs. Willie Thompson of Little Rock provided the supporting information to correct the birth certificate.
